An
operator leaves his hydraulic excavator with the engine running. He
wants to fetch a slinging clamp that was left unattended and puts it at
the foot of the operator's seat for transportation.

When
the operator put down the clamp on the floor, he placed it on the left
travel control pedal by mistake. That started the machine to move
backward and turn to the left. He was run over by the undercarriage and
killed.
